A leading healthcare system located in northern New Hampshire is now seeking an experienced Primary Care Nurse Practitioner to join their award-winning medical team!

This 25-bed critical access community hospital in New Hampshire’s Upper Connecticut River Valley provides comprehensive, patient-centered care, including 24/7 emergency services, inpatient and outpatient surgery, primary and specialty care, diagnostic imaging (CT, X-ray, ultrasound, mammography), on-site laboratory testing, and rehabilitation therapy, with convenient telehealth and coordinated care transitions. Join our team as a Primary Care Nurse Practitioner and deliver high-quality, patient-centered care in a collaborative rural outpatient setting, providing comprehensive assessments, wellness visits, chronic disease management (diabetes, hypertension, COPD), acute care for minor illnesses and injuries, preventive screenings, vaccinations, medication management, and evidence-based treatment plans. Daily responsibilities include conducting thorough histories and physicals, ordering and interpreting labs and diagnostic imaging, e-prescribing, coordinating care with physicians and specialists, triaging patient concerns, managing refill requests, documenting efficiently in the EHR, delivering telehealth visits, educating patients on lifestyle and self-management, and contributing to quality improvement and population health initiatives.

What's Needed to Succeed:

  • Advanced degree from an accredited Nursing program
  • Licensed as a Nurse Practitioner APRN in the state of New Hampshire or ability to obtain
  • Prior working experience in a primary care, family medicine, or internal medicine setting
